HeartPath Home Care
Approved Georgia GAPP Provider

Your child may already qualify for skilled nursing at home.

Caring for a medically fragile child is a lot to carry — you don't have to do it alone.

Through Georgia's Pediatric Program (GAPP), medically fragile children can receive in-home nursing care from licensed RNs and LPNs — and for qualifying families, it may come at no cost through Medicaid.
